T ITLE OF INFORMATION COLLECTION: Generic Clearance for the Collection of Patient Experience Survey Information at Improving Patient Care Sites, OMB Form No. 0917-0036: Indian Health Service (IHS) Patient Experience Survey.


PURPOSE:

The information obtained from the customer satisfaction surveys provides feedback to the provider and care teams about patient’s experience of care. This information is used to make improvements in patients’ care at the facility.


DESCRIPTION OF RESPONDENTS:

The patient experience survey is made available to patients on a voluntary basis at the end of their visit. Patients are chosen randomly, and each site collects the information on approximately 15-20 patients per month. There is no personally identifiable information on the survey. There are 7 questions based on the patient’s experience at the facility. Users will need to record the total number of #4 (Agree) and #5 (Strongly Agree) responses to the single question #7 for Patient Experience onto the data template: “I receive exactly the care I want and need exactly when and how I want and need it.” IPC uses question 7 as an overall indicator of patient satisfaction, however responses to all the questions will provide teams with specific information about what the likes and dislikes of the patient’s experience and for teams to identify areas of improvement. These rates are reviewed for patient satisfaction and compared for trending analysis and improvement.

At the approximately 50 federal sites, one person spends about 5 minutes on each survey tallying the response rates, collecting the information and reporting the totals in a reporting template. If there are 20 surveys completed each month and 240 per year, which is about 20 hours each year. A GS 9 rate of about $20 per hour x 20 hours is $400 per year.


FEDERAL COST: The estimated annual cost to the Federal government is approximately $400 annually.
