Requested Revisions to the Confidentiality Statement for NAEP Teacher and School Questionnaires |
The purpose of this memo is to document the revision to the confidentiality statement that appears on the from cover of the NAEP teacher and school questionnaires in 2008. The revision results in consistent language across the student, teacher, and school data collection documents. Below is the revised text (with the original version for reference).
The original version of the teacher/school confidentiality statement has been printed on the covers of the students with disabilities (SD) and English language learners (ELL) questionnaires for the 2007-08 long-term trend assessment.

REVISED 2008 School [and Teacher] Confidentiality Statement
According to the Paperwork Reduction Act of 1995, no persons are required to respond to a collection of information unless such collection displays a valid OMB control number. The valid OMB control numbers for this information collection are 1850-0790. The time required to complete this information collection is estimated to average 30 minutes per response [for teachers, this would read “20 minutes per response”], including the time to review instructions, search existing data resources, gather the data needed, and complete and review the information collection. If you have any comments concerning the accuracy of the time estimate(s) or suggestions for improving this form, please write to: U.S. Department of Education, Washington, D.C. 20202-4651.
