cover letters to request evidence or request a signed Form
SSA-2935-U3 Authorization to the Social Security Administration to
Obtain Personal Information" from the
applicant/parent/guardian/custodian of record. We are making minor
changes to the forms SSA-9308 and SSA-9310. 1)We are changing Form
SSA-9308 to SSA-L2937. We are also removing reference to the 1-800
numbers, because several regional reviewers' toll-free numbers are
not 800 numbers. The change will allow regional reviewers to type
their specific toll-free number without making strikeouts in the
letter. 2)We are changing Form SSA-9310 to SSA-L2938. We are also
removing reference to the 1-800 numbers, because several regional

The decrease in the burden is
due to the fluctuation in the number of annual respondents. SSA
bases our estimates on systematic data, and we estimate the numbers
accordingly. In addition, we needed to revise this Information
Collection Request to show the categories listed in the chart above
(see #12). These categories have not changed since the last time we
received approval; however, we inadvertently did not create
separate ICs for each one. Therefore, we added several ICs to this
request to accommodate each line of the chart. The increase in the
cost burden estimate is due to fluctuations in costs, and inflation
rates.
