Obtain Personal Information" from the
applicant/parent/guardian/custodian of record. We are making minor
changes to the forms SSA-9308 and SSA-9310. 1)We are changing Form
SSA-9308 to SSA-L2937. We are also removing reference to the 1-800
numbers, because several regional reviewers' toll-free numbers are
not 800 numbers. The change will allow regional reviewers to type
their specific toll-free number without making strikeouts in the
letter. 2)We are changing Form SSA-9310 to SSA-L2938. We are also
