Disability Program Feedback Survey
Survey - The Disability Awareness Campaign!
Dear Colleague:
I am excited to inform you that Social Security is planning to launch the “Faces and Facts of Disability”
Awareness Campaign.
The objective of the campaign is two-fold:
•
•
To communicate the importance and benefits of the disability programs; and
To provide the facts and correct misinformation.
As we work to develop public information materials to share with you and your constituents, we need
your help! We have a couple of questions to gather your feedback about Social Security’s Disability
programs. Please share this survey (link) with your constituents.
We value your thoughts and look forward to hearing from you.
Thank you for your continued support of Social Security.
Sincerely,
Kojuan Almond
Associate Commissioner
