The School Data Form is part of the data collection for The National Title I Study of
Implementation and Outcomes: Early Childhood Language Development (ECLD), sponsored by
the Institute of Education Sciences (IES) in the U.S. Department of Education. ECLD seeks to
identify school programs and teacher instructional practices associated with improved language
development, background knowledge, and comprehension outcomes for children in
prekindergarten through third grade.

We are asking you to provide the requested information on each of the schools listed in the
attached form. The data collected from this form will be used to confirm the study eligible
