There is no set form (recordkeeping activity) for the school district staff to record information on parents’ preferred providers. The information is requested to be provided in an electronic format – but the program will accept the information in any format. The program sponsor said they did not develop a specific form to record the information. However, if OMB wants a specific form, the contractor will develop one.
