The Robert C. Byrd Honors Scholarship Program Performance Report is submitted annually. This report will cover your FY 2006 allotment for scholarship awards between July 1, 2006 through June 30, 2007.

Section 1 – Budget summary:
Enter the amount of funds and the number of recipients for each category, as specified.
Section 2 – Funds and recipients by type of postsecondary institution:
Enter the total number of scholarship recipients and the total amount of program funds awarded to those recipients for attendance at public, private and proprietary in-state, and out-of-state institutions of higher education.
Section 3 – Number of graduates in this report period:
Enter the number of students who graduated during the report period after four years or less of postsecondary enrollment and enter the number of students who graduated during the report period after more than four years of postsecondary enrollment.
Section 4 – Scholarship recipient outcomes and status in this report period:
Enter the number of scholarship recipients that are designated by the postsecondary institution to be of a first-year, sophomore, junior or senior standing and that meet the following descriptions:
Received full $1500 scholarship
Graduated before the end of the report period – Students who graduated after receiving less than the full $1500 scholarship during the report period
Received scholarship after first semester or quarter – Students who did not receive scholarship funds in the first semester(s) or quarter(s) of the report year
Less than full-time status
Postponed or interrupted scholarship – Students who were permitted to postpone or interrupt enrollment to pursue other opportunities such as an internship, mission, study abroad, etc. for at least one semester or quarter during the report year
Number of students with other reasons
Failed to submit required paperwork – Students who failed to submit the required paperwork to prove academic standing and thus did not receive scholarship funds for at least one semester or quarter of the report year
Dropped out of school
Academic progress inadequate to maintain scholarship – Students who failed to maintain satisfactory progress as determined by the institution of higher education
Received more than $6,000 or exceeded time – Students who were no longer eligible to receive scholarship funds in the final semester or quarter(s) of the report year because they exceeded the maximum time or funds (exclude graduates). Students may receive scholarship funds for a maximum of four years or receive up to $6,000.
Section 5 – Students No Longer Financially Eligible:
Provide the number of students who received the full $1500 scholarship, but will no longer be eligible to receive a scholarship in the next report period due to receiving more than $6,000 or have exceeded the four-year maximum scholarship term (exclude graduates).
Students may receive scholarship funds for a maximum of four years or receive up to $6,000. Some students will not have graduated before exceeding the maximum time or funds. Enter the number of students who received the full $1500 scholarship during the reporting year, but will be ineligible for a Byrd scholarship in the next reporting year.
Section 6 – Certification by Authorized Agency Official:
An authorized official of the State educational agency that has a legal agreement with the U.S. Department of Education to participate in the Robert C. Byrd Honors Scholarship Program must sign the certification statement. (NOTE: Since the State educational agency is the entity that has legal responsibility for administration of the program, an authorized official of that agency must sign this statement even if the State Educational Agency has delegated certain administrative responsibilities to another agency.
