subject: Change Request: Impact Evaluation of Upward Bound’s Increased Focus on Higher-Risk Students – Baseline Data Collection Protocols (1850-0822)

We are writing to request changes to the following forms approved for use by the Department of Education for the above data collection (OMB Control Number 1850-0822):

• Attachment 1 (Parental Consent Form) – changes to provide greater clarity and legal references, as requested upon detailed review of the consent form by attorneys in the Office of the General Counsel at ED

• Attachment 2 (Student Informed Assent Form) – changes to provide greater clarity and legal references, as requested upon detailed review of the assent form by attorneys in the Office of the General Counsel at ED

• Attachment 3 (Baseline Information Form) – change in the response option for Question 24, regarding participation in other programs, to indicate the student’s time in each other program (under 1 month, 1-3 months, or over 3 months), instead of the starting and closing dates of student participation, since the student’s accumulated time in another program is the most relevant information for this study

We have provided electronic versions of these documents tracking the changes from the versions of the forms approved by OMB.

We do not believe that these changes have any effect on respondent burden under this data collection.
