Abstract: The current approved data collection instrument contains 68 questions, and this request if approve will decrease the number to 62 questions. Some questions have been deleted and others replaced with new questions. There is no change in the annual reporting or recordkeeping cost burden. The question covered by this change request for the new data collection instrument are: Questions 1-2, 48; REVISED to allow ability to provide more detailed response as requested by respondents. Questions 3-4, 26, 45 ; PREVIOUS question from past instrument. Questions 5, 8-15, 17, 27, 31-32, 42-44, 46-47, 50, 60-62; NEW. Questions 16, 30; REVISED for clarification and reduction of information to be recorded. Text boxes provided to allow comments regarding each section of the instrument. Current Instrument: Questions 1-2, 15, 23, 27-28b, 54-55; UPDATED. Questions 5-10, 13, 16, 18-20, 28c, 30-32, 34-40, 47-54; DELETED.
