DASIS is conducted to address the
mandates of section 505 of the Public Health Service Act for annual
information about public and private substance abuse treatment
providers and the clients they serve. Approval is requested for
identification of new treatment providers, updated information on
currently identified providers, and a questionnaire used for the
annual survey of all identified providers. The annual survey
questionnaire has been significantly shortened.
On behalf of this Federal agency, I certify that
the collection of information encompassed by this request complies
with 5 CFR 1320.9 and the related provisions of 5 CFR
1320.8(b)(3).

the proposed collection of information, that the certification
