DASIS is conducted to meet the
specific mandates of section 505 of the PHS Act for annual
information about public and private substance abuse treatment
providers and the clients they serve. Approval is requested for
identification of new treatment providers, update of information on
currently identified providers, and for a questionnaire used for
the annual survey of all identified providers.
On behalf of this Federal agency, I certify that
