Abstract: THE INFORMATION COLLECTED ON THIS FORM IS USED TO DETERMINE THE STUDENT STATUS OF CHILDREN OF COAL MINERS OR THEIR WIDOWS, AND CERTAIN BROTHERS AND SISTERS OF DECEASED COAL MINERS. THE FORM IS SENT TO THE BENEFICIARIES 6 MONTHS BEFORE THEIR 18TH BIRTHDAY IN ORDER TO DETERMIN IF THEY MEET THE REQUIREMENTS FOR CONTINUED PAYMENT OF BENEFITS BEYOND AGE 18.
