Abstract: THE INFORMATION COLLECTED BY USING THIS FORM IS USED TO DETERMINE THE STUDENT STATUS OF CHILDREN OR BROTHERS ENTITLED TO BENEFITS UNDER THE FEDERAL MINE SAFETY AND HEALTH ACT. THE FORM IS SENT TO THESE BENEFICIARIES SHORTLY BEFORE THEIR 18TH BIRTHDAY. THE FORM ADVISES BENEFICIARIES OF THE STUDENT REQUIREMENTS WHICH MUST BE MET FOR CONTINUANCE OF BENEFITS UPON ATTAINMENT OF AGE 18 AND CONTAINS QUESTIO WHICH MUST BE ANSWERED AND RETURNED TO SSA BEFORE AGE 18.
