This standard Government form, prescribed by OMB Circular A-102, is used to apply for assistance under this program. Applicants use this form to outline scope, costs and expenses for the project. (0572-0096)
There are two attachments to the SF-424. One is a Site Worksheet (required) and the other is a Survey on Ensuring Equal Opportunity for Applicants (optional).
