The Personnel Vetting Questionnaire is an information collection completed by applicants for, or incumbents of, Federal Government civilian or military positions, or positions in private entities performing work for or on behalf of the Federal Government under contract. The collection is used as the basis of information by the Federal Government in conducting personnel vetting background investigations, reinvestigations, and continuous vetting, as appropriate. It is also used by agencies in determining whether a person performing work for or on behalf of the Federal Government under a contract should be deemed eligible for logical or physical access for the nature of the work. In most cases, this collection is completed once a conditional job offer of employment is provided to the respondent.
