Circumstances of Change Request for OMB 0920-1383


The Centers for Disease Control and Prevention (CDC), Division of Global Migration Health (DGMH) requests a nonmaterial/non-substantive of the currently approved Information Collection Request: Animal Imports (OMB Control 0920-1383).


Change Request

CDC is requesting a non-substantive change to the CDC Dog Import Form. The CDC Dog Import Form is a free, online form that anyone bringing a dog to the United States must complete. Upon successfully completing the form, importers will automatically receive a receipt that they must present to their airline prior to boarding and to government officials upon arrival in the United States. They may either present the receipt as a hard copy or digital copy from a phone or other electronic device.


CDC proposes adding the following question to Section C: Travel Information of the CDC Dog Import Form:

  • Does the dog have a reservation at a CDC-registered animal care facility?

    • Response options: Yes/No


CDC does not consider this change substantive since the addition is one question. CDC does not anticipate this change will add to the estimated time it will take for importers to complete the CDC Dog Import Form. The addition of this question aligns with required documentation to bring a foreign-vaccinated dog to the United States from a high-risk rabies country.




Increase in Annualized Total Burden Hours:


The previous burden calculated for this data collection form consisted of 87,500 hours (rounded to nearest hour). CDC does not anticipate this addition will change the estimated annual burden.
