Mariner PrepEx Survey
This mariner survey is part of the U.S. Department of Transportation's Maritime Administration's Mariner
Preparedness Exercise Program. Please complete and submit this survey within 10 days of this email.
This request is in accordance with the Paperwork Reduction Act of 1995. The information you provide will be
used in strategic planning of crew training and certifications, and vessel activation policies. The OMB Control
Number for this information collection is 2133-0550. The estimated average burden associated with this
collection of information is approximately 5 minutes per response, including the time for reviewing
instructions, completing, and reviewing the collection of information. All responses to this collection of
information are voluntary. Comments regarding this burden estimate or any other aspect of this collection of
information, including suggestions for reducing this burden to: Information Collections Clearance Officer,
Maritime Administration, MAR-390, 1200 New Jersey Avenue SE, Washington, DC 20590. An agency may
not conduct or sponsor, and a person is not required to respond to a collection of information unless it displays
a current valid Office of Management and Budget control number.
