Q1.1 FLASH FLOOD RESPONSE SURVEY

Thank you for helping us understand how people respond to hazardous weather!

This brief survey asks about your experience during the {DATE} weather event in your area. Your responses will help improve how weather alerts and information are delivered to the public. It should take 5–10 minutes to complete.

Your participation is:

  • Voluntary – You can skip any question or stop at any time.

  • Anonymous – We will not collect your name or contact information.

  • Confidential – We will not reveal any personally identifiable information.


There is no risk or penalty if you choose not to take part. We are not offering any payment for participation. Results may be shared with local officials and made publicly available, but without any identifying details.
