REPORTING FORM INSTRUCTIONS
Please read instructions carefully. Reporting period is 1/1/2026 – 12/31/2026
This form is to be used to report the number of fish discarded and protected species gear interactions.
Please complete the form as thoroughly as possible and include it with the SE coastal fisheries
logbook form for the corresponding trip.
All forms must be submitted no later than seven (7) days after the completion of each trip (the date of
landing). Completed forms are to be mailed to:

Please print all requested information clearly. A form with incomplete or unclear information cannot be
entered into the database and will be returned. A missing form may cause you to be out of compliance
and your permit renewal denied.
Please enter the same Vessel Number and Trip Start Date that are on the corresponding logbook
trip report form. Do not send a discard form for months you did not fish.
You must fill out a SEPARATE form for EACH fishing trip made. DO NOT report multiple trips on one
form. All information for one fishing trip should be entered on one form. There are two sections for
each form. The top of the page, or Vessel Section, is to help us link this discard report with the SE
coastal fisheries logbook trip report form (OMB#00648-0016). The bottom portion of the page, or
Discard Section, is used to report your interactions with species which were caught, or entangled in
gear, but are not kept and brought back to shore.
SE VTR# - Please provide the SE VTR# from your corresponding SE Coastal Fisheries Trip Report
Form. Do not change, modify or create this number.
If no discards or protected species gear interactions were encountered on a trip, please check the “No
Discard Interactions this Trip” box on the form.
Vessel Section - Fill in each information block on each log as described below:
Captain’s Signature - The person responsible for the operation of the vessel during the
trip must sign the form to verify all information.
Phone No. - Include a phone number where you can be reached.
Vessel ID. No - Enter the official US Coast Guard documentation number or state
registration number for the vessel as it appears on the permit application.
Vessel Name - Enter the vessel name as it appears on the permit application.
Trip Start Date - Enter the date that the vessel left the dock for this trip (this should be
the same start date as reported on the logbook form).

Discard Section - Report each species discarded per gear type and, if applicable, each protected species
which interacted with your gear on this trip. You may report species on more than one line if the gear
type of condition type changes.
Species - Print the name of all fish, marine mammals or turtles that was discarded or that
interacted with your gear.
Number - Report the estimated number of animals for each species discarded or
encountered (caught with hooks, tangled in lines/nets, etc.).
Average Weight - Report the estimated average individual weight in whole pounds for
each species discarded.
Discard Condition - Enter the following codes for the condition of the discards or
protected species interactions:
1 - DEAD ALL: All discards were dead (floating on the surface).
2 - DEAD MAJORITY: More than 50% of the discards were dead.
3 - ALIVE ALL: All discards were released alive at the surface without the use
of a descending tool (swam away when returned).
4 - ALIVE MAJORITY: More than 50% of the discards were released alive at
the surface without the use of a descending tool.
5 – KEPT: Catch was kept - not sold (i.e., it was eaten, or used as bait by crew).
6 - Unable to determine.
8 – Descending Tool: 100% fish released at the bottom with descending tool
Discard Reason - Enter the following codes for the reason of the discards or protected
species interactions:
A – REGULATIONS – NOT legal size: Animals that would have been sold,
however local or federal size limits forbid it.
B – REGULATIONS – Out of season: Animals that would have been sold,
however the local or federal fishing season is closed.
C – REGULATIONS – Other: Animals that would have been sold, however a
local or federal regulation, other than size or season, forbids it (Other than size
or season; i.e. protected species, not properly permitted).
D – MARKET CONDITIONS: Animals that have no market value (rotten,
damaged).
Gear – Enter the appropriate gear code for the type of gear (‘T’, ‘L’, ‘GN’, ‘H’, ‘E’,
‘TR’, ‘B’, ‘S’, ‘P’, ‘O’) used during each species interaction. If the same species were
encountered using multiple gears, please enter the number of discards encountered
separately for each gear type. See the Gear Section in the instructions of the Coastal
Logbook (pg. 2-3) for more information on Gear Codes.
Area - Enter the numeric code for the area where the discarded species were caught.
Maps with numeric codes and associated latitudes and longitudes are on page 3 of these
instructions.
* Do not use state codes or LORAN coordinates.
Depth in Feet – Enter the bottom depth (in feet) where the majority of each species was
caught.
* Do not record depth in fathoms or meters.

Public reporting burden for this collection of information is estimated to average 15 minutes per response including the time for reviewing
the instructions, searching the existing data sources, gathering and maintaining the data needed, and completing and reviewing the collection
of information. Send comments regarding this burden estimate or any other aspects of this burden to Dr. Dave Gloeckner, National
Marine Fisheries Service, 75 Virginia Beach Drive, Miami, Florida 33149. This reporting is required under and is authorized under 50 CFR
Parts 622 and 635. Information submitted will be treated as confidential in accordance with NOAA Administrative
Order 216-100. Notwithstanding any other provision of the law, no person is required to respond to, nor shall any person be subject to a
penalty for failure to comply with, a collection of information subject to the requirements of the Paperwork Reduction Act, unless that
collection displays a currently valid OMB Control Number. The NMFS requires this information for the conservation and
management of marine fishery resources. This data will be used to monitor the quota for this fishery.
Privacy Act Statement –
Authority: This information collection is authorized under the Magnuson-Stevens Fishery Conservation and Management Act (16 U.S.C
1801 et seq.), the High Seas Fishing Compliance Act, the American Fisheries Act, and the Atlantic Coastal Fisheries Cooperative
Management Act.
Purpose: To manage U.S. fisheries, NMFS requires the use of registrations by participants in the United States. Information on NMFS
forms can include permit holder or vessel owner contact information and vessel descriptive information, and may be used as part of fishery
management council analyses to inform and support decisions. The data collected will be used to estimate non-reporting of fishing trips and
reporting errors. This information will allow NMFS to track catch and effort of federal fisheries permit holders, to ensure adherence to
regulations and maintenance of species quotas, with the overall goal of maintaining and/or achieving sustainable fishing for all federally
managed species.
Routine Uses: The Commerce Department will use this information to ensure compliance with the regulations at 50 CFR part 622.
Disclosure of this information is permitted under the Privacy Act of 1974 (5 U.S.C. Section 552a), to be shared among NMFS staff to
coordinate monitoring and management of sustainability of fisheries and protected resources, as well as with the applicable state or regional
marine fisheries commissions and international organizations. Disclosure of this information is also subject to all of the published routine
uses as identified in the Privacy Act System of Records Notice COMMERCE/NOAA-6, Fishermen's Statistical Data, and COMMERCE/
NOAA-19, Permits and Registrations for the United States Federally Regulated Fisheries.
Disclosure: Furnishing this information is mandatory. Making a false statement on these forms is punishable by law. Failure to provide
complete and accurate information will prevent the determination of compliance and eligibility to obtain benefits.

Statistical Grid Map – Grid numbers follow lines of longitude and latitude*. The first two digits in the
four digit grid numbers are latitude degrees, and the second two digits are longitude degrees.
*NOTE for Florida Keys Areas – Statistical grids 2481 and 2482 have been split along the border
between the Gulf of America and South Atlantic regions. The areas NORTH of the Florida Keys are
to be reported as historical areas 1 or 2. Areas SOUTH of the Florida Keys are to be reported as areas
2480, 2481, or 2482.
