Dear [RECIPIENT NAME],
We are contacting you again to request your participation in a USDA/APHIS/Wildlife Services survey study about
predators and predator damage management in your state. You are among a select group of livestock producers
whose input will help Wildlife Services improve their strategies for predator damage management in your state.
This questionnaire provides an opportunity for your voice to be considered. We recognize this is a busy time of
year, but your input is vital! The questionnaire should take no more than 45 minutes to complete.
To complete the survey, simply click on the following link: [SHORTENED SURVEY LINK]
When prompted, enter this code in all caps: [UNIQUE CODE]
The online questionnaire accessed with this code should not be shared with others as it will give them
access to your responses. If you have trouble accessing the online questionnaire, please e-mail us at:
HDsurveys@usda.gov. Additionally, if you are unable to complete the survey online, please let us
know and we will send a paper copy of the questionnaire to you in about two weeks.
We will never reveal your identity or any information that could be used to identify you. When we report or share
data with others, we will combine demographic data from everyone who participates, and we will never reveal
potentially identifying information about anyone who fills out the questionnaire.
If you have any questions, please do not hesitate to contact the project coordinator, Hailey Ellis, via email at
hailey.e.ellis@usda.gov or by phone at (970) 266-6148.
Sincerely,

Hailey Ellis
