management in your state. The questionnaire should take no more than 45 minutes to complete. If you already

This survey study is being conducted by researchers at USDA/APHIS/Wildlife Services, and you are among a select
group of livestock producers invited to share your perspectives and help Wildlife Services improve their predator
damage management efforts. Sharing your perspectives is crucial to obtaining representative participation of
livestock producers.
To make it easier for you to fill out, the questionnaire is designed to be taken online. To complete the

You will need to type the survey website address in your web browser (for example, Internet Explorer, Mozilla
Firefox, Safari, Chrome) exactly as spelled out above. Detailed instructions on how to access the online
questionnaire are printed on the back of this letter. You cannot get to the questionnaire website by typing it into
a search engine such as Google or Yahoo.
Once you have gotten to the website that hosts our questionnaire, you can begin taking the

This code is unique and should only be used by you to access and complete the questionnaire. If the code is
