This is a request for OMB to approve a non-substantive change to the attached CTSU OPEN Rave
Request Form. The update to the CTSU OPEN Rave Request Form is as follows:

Updated section 2.8 with the removal of list for the Type of Credentialing to allow for it to
be entered as free text.

The update to question 2.8 will not increase the burden to complete the form.
Attached to this request is the revised form (clean and tracked versions). All the changes can be
seen in bold red (additions and strikes) in the “Revised Marks” file.
