Public Burden Statement: The overall purpose of this study is to conduct an independent survey of public opinion regarding various issues related to organ donation. This Data is essential for planning, targeting, and implementing outreach efforts to increase public donation commitment as well as for tracking the results of such efforts over time. An agency may not conduct or sponsor, and a person is not required to respond to, a collection of information unless it displays a currently valid OMB control number. The OMB control number for this project is 0915-0290 and it is valid until XX/XX/202X. This information collection is voluntary. Data will be private to the extent permitted by the law. Public reporting burden for this collection of information is estimated to average 17 minutes (0.29 hours) per response, including the time for reviewing instructions, searching existing data sources, and completing and reviewing the collection of information.
