Attached is the Information Collection Request (lCR), CJIS Biographic Verification Request Form (1-791)
for approval and submission to the Office of Management and Budget (OM B). The requirements of this
collection are prescribed by Title 28, Code of Federal Regulations, Sections 0.85, Section 20.33, and 50.12.
The necessity of this collection ensures individuals with poor quality fingerprints, or are pennanently
unable to provide fingerprints at all, are not denied benefits, licensing, or employment opportunities.
