eLAS Screens for Initial Claims
Protective Filing Information Screen

We collect the individual personal information and ask them if they want to schedule an
appointment. If they answer “yes” we ask for their prefer method for the appointment (i.e.,
phone, in-office, or video) for filing an initial claim application. We use this information to
create a record of their intent to file for benefits and start the process to schedule an appointment.

Contact and Communication Information Screen

We collect the contact information as mailing address, phone number, and Special Notice
options to provide confirmation for scheduled appointments.
