Survey Introduction

Dear [State Coordinator’s name],

Welcome to the 2024 State Education Agency (SEA) Title IV, Part A Needs Assessment Survey (survey). On behalf of the U.S. Department of Education’s (ED) Office of Safe and Supportive Students (OSSS), the Title IV, Part A Technical Assistance Center (T4PA Center) requests your participation in this year’s survey to better understand how the T4PA Center and ED can support State Coordinators’ (SC) current training and technical assistance (T/TA) needs, learn if and how SCs are providing T/TA to local education agencies (LEAs), and receive feedback about the T/TA presented by the T4PA Center since the previous survey in early spring 2023.

The information collected through this survey will be used to: 1) update the data-driven T/TA plans for each SEA for the upcoming fiscal year, 2) evaluate the quality and usefulness of the T/TA provided by the T4PA Center over the past year, and 3) inform the development of Title IV, Part A-specific resources, training, and other materials and methods of technical assistance the T4PA Center and ED will provide to SCs in 2024 and beyond.

The staff at the T4PA Center thank you in advance for completing this brief survey. We estimate it will take approximately 15 minutes to finish. Please submit your responses as soon as possible, but no later than 9:00 p.m. ET, on April 19, 2024.

To begin, please click on the “Next” button at the bottom of the screen. You may leave and return to the survey at any point; your answers will be saved.

Sincerely,

Title IV, Part A Technical Assistance Center

Survey Text

Thank you for participating in the 2024 SEA Title IV, Part A Needs Assessment Survey. The T4PA Center seeks to better understand your program through this annual survey. The information you provide will give us critical insight on the topics and types of technical assistance that is most helpful to your success in administering the Title IV, Part A program. We thank you, again, for taking the time to respond.

Survey Overview

The survey has five topical categories: (1) Title IV, Part A program implementation, (2) well-rounded educational opportunities, (3) safe and healthy students, (4) effective use of technology, and (5) support provided by the T4PA Center. In each section, we seek to understand more about the T/TA you provide to LEAs and the areas in which you need TA support from the T4PA Center.

Throughout the survey, you will encounter several question types, including multiple choice, checklists, and open-ended responses. A limited number of text boxes will require written responses. We estimate it will take approximately 15 minutes to complete. You may leave the survey at any point; your answers will be saved. Please use the link in your invitation email to return to the survey.

Uses of the Data

The information you provide in this survey is collected solely to inform the technical assistance provided by the T4PA Center. Your responses help customize the support you receive and allow us to track whether we, as a center, are meeting your needs.

If you have any questions about how your data is used or if you need assistance completing the survey, please contact the T4PA Center at the following email or toll-free number: info@t4pacenter.org or 833-404-4845.
