Dear Colleague:

The Office of Research Services (ORS), Events Management Branch asks for five minutes of your time to provide feedback on recent ASL interpreting services you have requested and/or received. As a key stakeholder invested in the outcomes that ASL Interpreting Services delivers, your feedback is a valuable piece of information that can be used to improve our services. Your feedback will be combined with others, analyzed by the ORS Office of Quality Management (OQM), and will remain confidential.

NOTE: This survey must be completed in one sitting. If it times out or you exit the survey before you have finished answering the questions, you will have to start over.
