Information is collected on the DD
Form 2970 as a one-time submission by a food vendor requesting to
operate a food establishment on a military installation. Approval
to operate the food establishment is determined by the
installation's medical authority; preventive medicine (public
health) uses the information as part of a pre-operational
assessment to determine if the vendor meets prescribed DoD food
safety requirements. Food vendors who are deemed inadequately
prepared or equipped to provide safe food service are disapproved
for operating on the installation.
