Dear Transplant Consortium Member:
The Patient Education and Support Committee is gathering information about survivorship concerns
across interdisciplinary teams. Often, these conversations occur during Day 100+ post-transplant
follow-up visits. However, given the specific needs of the individual and your specialty, you may touch
on them during pre-transplant visits or the transplant hospitalization.
This survey is organized by body system, organs and psychosocial concerns with three questions
associated with each: is the topic addressed, your comfort addressing the topic, and desire for patients
to have additional education or resources. Given your professional discipline, some areas may not be
applicable to your discussions with patients. Simply click N/A for those items. A Likert scale is used to
answer the questions. Space for comments is available for each topic area.
The final question indicates your interest in participating in the development of patient education and
resources per topic area. We recognize that your availability may be limited. Indicating ‘Yes’ does not
obligate you to future meetings. You may be able to act as an intermittent consultant. We will gather
that information when following up with you.
