Revisions to the Collection Instruments

SSA is making the following revisions:

• Change #1: We are removing SSA-1398 from this Information Collection Request (ICR).
  
  

Justification #1: We no longer ask Ticketholders to provide earnings from Self Employment; therefore, this form is obsolete. We will not collect this information through any other means.

• Change #2: We are removing SSA-1399 from this ICR.

Justification #2: We no longer have Universal Autopay; therefore this form is obsolete. We will not collect this information through any other means.

We will remove these forms from the ICR upon OMB’s approval. In the meantime, we are not using them to collect information. Since our usage of these forms was already low, which is one of the reasons we are discontinuing their use, we do not believe the removal of these forms will change the overall burden for this collection.
