Study Team: I received your name and contact information from Doctor [referring doctor’s name] who said you were interested in learning more about this study on using technology for pain management. I’d like to tell you more about this research study, and then ask you a few questions to see if you are interested and eligible to participate in the study. Is this an ok time for you to speak?

If the Person says “No” or “I’m not sure”

Study Team: Okay. [Ask if you can schedule another time to talk. If the person is not sure or seems hesitant, thank him/her and say goodbye.]

Study Team: Great. I wanted to let you know about a new study for patients who experience pain led by Dr. Urmimala Sarkar. We would like to collect patient feedback about the use of technology, like virtual reality, to manage pain. Would you like to hear more about this study?

If the Person says “No” or “I’m not sure”

Study Team: No problem. [If not sure] You can let me know if you have any questions that will help you decide, otherwise you can hang up. Thank you for your time.

If the Person says “Yes”

Study Team: Ok. I can tell you more about the study and then will ask you a couple questions to see if you are eligible to participate.

People who volunteer to be in this study will be asked to participate in an interview and provide feedback on a virtual reality headset by trying it. The study will take place in private offices at Zuckerberg San Francisco General and will take about 60 minutes to complete.

We will do our best to keep all of your information confidential. However, we cannot guarantee total privacy.

Being in this study is optional, and if you decide to participate, you can stop being in the study at any time.

I would like to ask you some questions to find out if you are able to participate in the study. Some of the questions ask about health conditions. Can I ask you these questions?

If the Person says “No”

If the Person says “Yes”

Study Team:

1. Do you have a history of seizure disorder?

2. Do you have a history of vertigo?

3. Do you have a history of claustrophobia?

If the Person answers “Yes” to one or more questions

If the Person answers “No” to all questions
