Minor Revisions to the Collection Instrument
SSA is making the following revision:
Change #1: We are revising Section E on pages 4, 6, and 8 from “E,” to “E.” with a period instead of a comma.
Justification #1: This is a typographical error; the comma should never have been there in lieu of the standard period that exists at all the other lettered sections.
Change #2: We are revising the Privacy Act Statement on this form.
Justification #2: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. As a result, SSA is updating the Privacy Act Statement on the form.
