You recently received an email from Charlie Homer and Mark Greenberg of the U.S. Department of Health and Human Services (HHS) and Tracy Wareing-Evans of the American Public Human Services Association (APHSA) asking you to participate in the State Questionnaire of Integration of Health and Human Services Programs. However, please ensure that you coordinate across potential respondents and submit only one response from your state through the link provided above.
