In addition to their roles in advising the project, Dr. Balaban will lead the data collection at Cambridge Health Alliance, Dr. Palen will lead the data collection at Kaiser Permanente Colorado, and Dr. Hochman will lead the data collection at AltaMed.
Technical Expert Panel (TEP)
JSI established a Technical Expert Panel (TEP) to provide expert advice on how to develop a REV to better deliver care to recently discharged hospital patients. The TEP will provide guidance at several points throughout the project, such that they will be able to offer input on all the major tasks contributing to the development of the REV: the Environmental Scan, Analysis of Primary Care Processes, Identification of REV components, and dissemination of the project’s deliverables and findings. The TEP is made up of ten individuals:
