Attachment 5: Teacher Script for Youth Survey
Dear Teachers,
Thank you for helping administer the youth survey as part of the Substance Abuse and Mental Health Services Administration (SAMHSA) underage drinking prevention initiative. The survey administration should take approximately 10 minutes to complete. Below we have included a brief script for you to read to your class prior to administering this survey. Although the script is a guide, we encourage you to adhere to the script in an effort to ensure that all important information in communicated to students. Thank you!
To read/speak aloud to students:
“Shortly, I will be passing out a brief survey. This survey is part of a larger initiative to address underage drinking. The questions are anonymous, so please do not put your name on this survey. Your participation is voluntary. Therefore, if for any reason, before, during, or upon survey completion, you may choose to withdraw from the study and your answers will not be included. If you have any questions or concerns about the survey or find that the survey topic triggers feelings or concerns, please let me know and I will connect you with appropriate supports.”
Teachers: Please collect all surveys upon completion and place in the envelope provided.
