This questionnaire is part of the Primary and Behavioral Health Care Integration (PBHCI) Evaluation, a national evaluation being conducted for the Substance Abuse and Mental Health Services Administration (SAMHSA) by Mathematica Policy Research. The questionnaire asks about your role and responsibilities, training, providing care, experiences integrating care, and staff interaction.
