This study is funded by the U.S. Food and Drug Administration’s (FDA’s) Center for Tobacco
Products (CTP) and conducted by RTI International. For this study, we would like you to
complete a survey about tobacco products. Because the survey asks a few questions about
your use of tobacco products, you may or may not feel uncomfortable answering such
questions. Your participation in this research study is completely voluntary, and you may
skip any questions you do not want to answer. Your identity will be kept anonymous, and no
one will be able to identify you as a participant in this study. Each survey will take no more
than 15 minutes to complete.

This study is funded by the U.S. Food and Drug Administration’s (FDA’s) Center for Tobacco
Products (CTP) and conducted by RTI International. For this study, we would like you to
complete a survey that asks teenagers what they think about cigarette smoking. About 600
teenagers will complete the survey, which takes about 15 minutes to complete.
Your participation in this research study is completely up to you. The survey asks questions
about your experiences and thoughts regarding cigarette smoking; however, you may skip
any questions you do not want to answer. During the survey, we do not ask for your name;
therefore, your name will not be connected to your answers. Furthermore, we will not share
any information you provide in the survey with anyone outside the research team, including
your parents.
To ensure your answers are kept private, please complete the survey in a place where no
one can look over your shoulder and view your answers. Also, please complete the survey in
