Thank you for participating in our short data collection on Customer Satisfaction of the CDC Newborn Screening Quality
Assurance Program. We would like to obtain your input regarding the service that you recently experienced in support of our
Quality Control (QC) and/or Proficiency Testing programs. This information will be used to identify areas of improvement.
Public reporting burden of this collection of information is estimated to average 10 minutes per response, including the time for
reviewing instructions, searching existing data sources, gathering and maintaining the data needed, and completing and
reviewing the collection of information. An agency may not conduct or sponsor, and a person is not required to respond to a
