Revision to the Collection Instrument

We are making the following revisions to the SSA-711 both to correct typographical errors, and in response to the public comment we received on June 22, 2015:

• Change #1: We are revising the numbering in the form, changing Section “8” to “7” and Section “9” to “8.”

Justification #1: The numbering on the form is incorrect, as it skips from Section 6 to Section 8. Therefore, we are correcting this typographical error.

• Change #2: In Section 5, we are making the following changes:

  • We are changing “Mother’s (Maiden) Name at birth (first, middle, last name) to Parent/Mother’s Name at birth (first, middle, last name);

  • We are changing “Mother’s married name(s)” to Parent/Mother’s married name(s) (if any);

  • We are changing “Father’s Name (first, middle, last name) to Parent/Father’s Name at birth (first, middle, last name).

Justification #2: We are making these changes due to public comments we received from Family Equality Council regarding non-gendered parental terms.

• Change #3: In Section 5 we are adding a request for “Parent/Father’s married name(s) (if any).”

Justification #3: We are adding this langauge due to public comments we received from Family Equality Council about non-gendered parental terms.

Upon OMB’s approval, we will also make these changes to the Internet version of the SSA-711.
