Thank you for participating in ‘Human Influenza Surveillance of Health Care Centers in the United States and Taiwan’ research study. This study asks that you return to the Johns Hopkins Emergency Department on ___________________, __________________ _______, ____ at _____: _____ am / pm.

This follow up visit should last no longer than 45 minutes. Research staff will collect 10 mL of blood from your arm as well as ask you a few follow up questions regarding your recent medical history.

We will compensate you $75 at the completion of this visit.

Instructions for your arrival visit:
Do not register yourself into the emergency department. Please come in and have a seat in the waiting room. Call a research coordinator at (443) 326 – 0534 to let them know you have arrived. A research coordinator will come get you in approximately 5 minutes.
