The paper form has been replaced with an
electronic format. States enter data directly into
the electronic form. The computer then formats the
information into a consumer-friendly display. The
electronic form is more convenient for states (e.g.
allows for annual updates only to make changes
rather than to redo the whole form), enhances
accuracy and tracking, and ultimately allows for a
better consumer experience.

form did not include this functionality.

Children's Dental Benefits Survey - The

electronic format includes drop-down lists in the
"frequency" field from which states can select the
appropriate option.

This functionality was added to streamline the data
entry process for states. States no longer need to
type in this information. They can just select from
the drop down list with a single click.
