Section 361 of the Public Health
Service Act (42 USC 264) authorizes the Secretary of Health and
Human Services to make and enforce regulations necessary to prevent
the introduction, transmission or spread of communicable diseases
from foreign countries into the United States. This is a request to
revise the data collection forms after extensive review. The
package includes forms for contact investigations of individuals
who may have come into contact with travelers that are confirmed to
have Ebola infection.

Extensive review of the data to
be collected resulted in changes to all forms, and burden
associated with illness tracking for illnesses approved in prior
version of this package has been slightly reduced as result.
However, the addition of Ebola forms has significantly increased
the burden (although this burden is included as a
contingency).
On behalf of this Federal agency, I certify that
