These forms are used for the Child
Support Enforcement Program under title IV-D of the Social Security
Act. With these forms, States report assorted administrative
expenditures made each quarter and the amounts collected as child
support payments each quarter. This reporting also includes a
breakdown of the collected payments that are distributed to the
custodial parent or to the Federal or State governments. These
forms also allow States to report the estimated amount of the
performance-based incentive payment earned each quarter and an
itemized rationale for the States balances of undistributed
collections. The expenditure and collection information is used to
calculate quarterly grant awards and annual incentive payments to
the States, and is published in an Annual Report available to the
general public. Respondents are limited to the designated child
support enforcement agencies in each State.
