Saliva Collection Instructions
In this packet you will find:
Two saliva collection kits with instructions
Two Saliva Sample Logs
Two smaller envelopes to place your saliva samples from each day
A larger, stamped envelope to send all your packets back in the mail
Instructions for returning the packet

What you need to do:
1. Collect your saliva samples and fill out the daily saliva sample logs

Instructions on how to collect the ten samples (5 samples per day) for the saliva part of the study are
included (yellow page). Follow these instructions and put your samples in the provided envelopes.

2. Mail back the packets

Before returning your samples, review the included checklist to make sure you didn’t forget anything.
Look for the page titled, “Instructions for returning your packet”.

Please try to complete this packet within a week. You will receive a reminder call from a clinic staff member to remind you
to collect the samples and return them in the self-addressed mailer within 8 days after your clinic exam. If we do not hear
from you first, we will give you a call at the end of the week and discuss your options for returning the packet even if you
do not wish to participate.
