Revision to the Collection Instrument
Change #1: We are removing questions #4 and # 7 from form SSA-4156.
Justification #1: Employers do not maintain this information or require it from the employee. The respondents do not complete these questions. Therefore, it is not necessary to include questions # 4 and #7 for the respondents on the form SSA-4156. Since the respondents have not completed these questions, the revision to remove the questions is minor.
Change #2: SSA is updating the Privacy Act Statement on the SSA-4156.
Justification #2: SSA’s Office of the General Counsel is conducting a systematic review of SSA’s Privacy Act Statements on agency forms. The current language, which dates back to the last reprint of the form, is now outdated.
Change #3: SSA is updating the PRA Statement on the SSA-4156.
Justification #3: We are revising the language to reflect our current, updated boilerplate language.
