Justification for Non-Substantive Changes to the Collection or Resubmission of the Collection within One Year of OMB Approval.
Change 1: We are deleting the question shown under the section titled “Change of Address” on the form SSA-1425. The question states the following:
“If Social Security sends your payments to your financial organization, do you want this to continue? YES or NO”
Justification 1: We received OMB approval for full clearance of OMB 0960-0073 on 12/12/12. In January 2013 we received policy feedbacks that a NO response to the question mentioned above has caused FO technicians to erroneously cancel direct deposit information from the beneficiary or recipient’s record. Due to the new direct deposit rather than paper checks requirement in 03/01/13, the volume of direct deposit-related requests from beneficiaries has increased. Form SSA-1425 is not usable for direct deposit changes because it does not have any area to house financial information. There is concern because of the inconvenience we have placed on the beneficiary or recipient when using the form, as well as an unnecessary increase in the FOs backlog in correcting the cancellation errors. This issue may result in creating a potential congressional inquiry or a public relations problem. Therefore, we recommend removal of the direct deposit question immediately instead of postponing until the collection is up for renewal. We will implement upon OMB’s approval.
