How do I know this is an official, approved DoD survey?
• Federal Voting Assistance Program (FVAP) is part of the DoD.
• In accordance with DoD Instruction 8910.01, all data collection in the Department
must be licensed and show that license as an Office of Management and Budget
(OMB) Control Number with an expiration date. The OMB Control Number for this
survey is xxxx-xxxx, expiring {EXPIRE DATE}.
• If you need verification that this is a legitimate survey, you can call 1-703-696-7427
for a list of current Defense Manpower Data Center (DMDC) surveys. This phone
line does not accept messages nor does it list surveys being conducted by any other
office.
