Attachment H: Screen Shots of the PRS

Following are screen shots of the HPOG Performance Reporting System, PRS. Baseline data collection will be conducted using this system. Individuals conducting intake will enter data about all study participants into the PRS. The system is currently being used for the Implementation, Systems and Outcome Evaluation of the Health Profession Opportunity Grants to Serve TANF Recipients and Other Low-Income Individuals. The Supplemental Baseline Questions will be incorporated into the PRS.
