NORC at the University of Chicago (NORC) is conducting a national survey of physicians on behalf of the Office of Minority Health. This survey collects information on physicians’ thoughts and opinions about racial and ethnic disparities in health care. The results will help fill in important knowledge gaps and will be valuable to OMH as they carry out their mission to protect and improve the health of racial and ethnic minority populations.
In a few days, you will receive a questionnaire asking your opinions on a variety of important health care issues. Your response is very important to the success of this study, and we sincerely hope that you will make the time and make your opinion heard.
We estimate that the survey will take about 15 minutes of your time to complete. Although your participation in this research is voluntary, your thoughts and opinions are extremely valuable to us. As a token of our appreciation for your participation, we will provide you with a check for $100. This check will be enclosed with the questionnaire we will mail to you next week. We will also be happy to provide you with a copy of the final report on the survey’s results if you would like.
You will have the option of completing the survey on paper and returning it in a pre-paid envelope or via a secured fax line. Or, if you prefer, you may call our toll-free number 1-877-236-8161 to schedule a telephone interview. Whichever method you choose, your responses will be confidential and your name will not be associated with the answers you provide.
